Imagine entering the rather small port of Venice, Italy aboard a 900 foot long cruise ship when the steering system fails. Despite the many safety mechanisms aboard the ship along with several tugboat tenders guiding it in to port, an investigation proved that once the steering system failed, the crew was not adequately trained in the emergency procedures for this situation and no one pushed the kill switch to shut off the engines and the ship rammed the dock, a private boat as well as a large ferry.
